Assistant randomly reading her instructions/not following prompt

I switched to Twilio numbers with all the v3 issues, and it was working much better. but I'm noticing on maybe 1 of every 10 calls she starts reading her instructions and not following her prompt. User answers and assistant says "Waiting silently for user to speak first" then just says random greeting rather than following the prompt..... anyone else?
