Custom Unsubscribe/Update/Amend Contact page

Perhaps this has been addressed here but I’m not able to find anything on how to do it. I’ve seen some GHLers create a custom unsubscribe link on their emails that takes the subscriber to a custom page/form where they can unsubscribe (of course) but also update or amend their info then resubmit. I used to use aweber and it had this functionality. Anyone know how this could be implemented? I think it would be extremely useful especially with DBR on an older list where updates are needed. Thank you.
