Contact button to open prefilled form?

Anyone know how to add a button to the contacts page to bring up a form with the contact's info pre-filled and ready to have notes, or updates to be done?

I know there's GHLPlugins (tried it a while back but they can't pre-fill the forms) and I know Extendly has the capability but I don't want to pay $500/mo for just that one simple thing.

I'm sure there's just some CSS code somewhere.

Any help would be appreciated.
