Chat GPT losing the thread and inconsistent output format

While I am able to have it create tables in a format I can upload for google responsive search ads, they often come out correctly the first 5-10 times, then chat GPT loses it.

Asking it to create a table output for responsive search ads with 10 headlines and 10 descriptions (accompanied with much more details on how these must be read in any combination, and combinable, character count, and examples). It will create a table output format for a while then randomly start returning it as text. Then when I stop it and ask it to refer back to the table format and the specific prompt outlining the output format it will continue to get it wrong. The character counts will become inaccurate, the ad language becomes single words, and the readability and usefulness of the headlines/descriptions will deteriorate.
