I usually do a 4 inch core through the body of the slab, This will usually allow me to confirm 1) is there a vapor barrier and 2) what is the condition of the existing vapor barrier. Many times I find the vapor barrier to be deteriorated to a point where it's usefulness is long gone.
NuCore Moisture Testing Requirements
They only call out ASTM 2659, that’s moisture meter guys. Here’s what it says: Although this floor is waterproof, from the top down, it is not to be used as a moisture barrier. The subfloor must be dry. Concrete moisture vapor emissions should not exceed 2.5% moisture content (ASTM F2659 method). Prior to installing flooring, remove dirt, paint, varnish, wax, oils, solvents, any foreign matter and contaminants from the subfloor. Do not use products containing petroleum, solvents or oils to prepare subfloors as they can cause staining and expansion of the new flooring. With the CMEX5, they have 2 scales that mimic the calcium carbide test method (sometimes referred to as the "bomb" test") - Carbide Method equivalent readings for concrete and anhydrite/gypsum substrates and screeds. Many manufacturers confuse everyone by calling out 2659 then referencing calcium carbide results.
