The FD 258 is an FBI form that’s used to collect fingerprints for background check purposes.Many states and agencies use this form to collect their own prints for background check purposes as well. The FD 1164 is the same type of from but it’s specific use case is for non US citizens to obtain their FBI background check through processing their fingerprints and cross referencing those prints through their criminal database. I just wanted to share this intake form I designed for my business. I wanted to give others ideas if they are new or looking for an option for their intake form. I incorporated suggestions Dan has given in the course and added some based on my specific Live Scan System. By no means is this the 'standard' I am sharing this in hopes it may help someone? 2-page document. Side 1 is for the customer. The other is for In Office Only. The customer will never see this form again, however, is used to add notes to prompt conversation for a thank you card or any type of follow-up that helps staff remember that customer specifically.
