Any known APP for community w/Member Dashboards + functionality to hold Live video group sessions?

Since my Ultimate Mission is to Empower each Client to become their own Doctor, I am looking for something uniquely powerful to offer people I help heal.

2 REQUIREMENTS FOR THE APP:

METRICS: There are many success metrics that are helpful to monitor when you are on your healing journey (For example: Blood markers, Symptoms (Physical, Mental, Energetic, Emotional), also key monthly milestones that are tied to their Life Design for the year). I would LOVE to find an APP that can provide customized dashboards per member. Right now I do it manually through Smartsheets (its not scalable) plus I want to empower each client, so they are able to easily build/manage their own. Examples/images in comments.
LIVE SUPPORT: I would like to use the same platform (if possible) to hold Live Video calls weekly with these members, where we can use their customize dashboards as reference to assess progress, and provide support in areas they are having challenges in. The real-time feedback can then be updated in their dashboards.
