When George asks, “does she walk around holding a pen that she never seems to need?” he’s referring to Bob Dole.
Dole always had a pen in his paralyzed hand. He lost full use of his right arm in a WWII mortar attack and his left arm wasn’t great either. Back when Seinfeld was on, he was a pretty big deal in politics, even ran for president.
