The 5 Whys
Today I had to take on an analysis of a problem at work. I utilized a tactic called the “The 5 Whys”. That’s where you identify the problem and then ask why it happened over and over until you get to the solution or root cause.
I find it useful when looking at events to determine a strong training solution (if required). Just thought I’d throw it out there!
