Niche within a niche?

Hello all,

I have an odd set of skills that I'm not sure if they merge on one channel. I"m a Naturopathic doctor, and I've been in oncology for the past 20 years along with general health issues (think family practice) and got burnt out. 8 years ago I shifted into fertility. Do I post just on fertility? Is that too small of a niche? Or do I go broad and general b/c as a primary care giver, I am well versed in several issues (ie) autoimmune disease. So confused what to focus on. I treat almost everything.... sigh...

Any help in orienting me in the right direction would be so appreciative.
