Lead Quality Problem ❌

For the last 2 weeks, I've been running ads for a cosmetic salon. The results of the campaigns are pretty good. I got her 42 leads with the average cost for each being $3. One lead has a value of $50 + they will come more than once.
The offer we run is one for lash lifting: buy 3 lash lifting treatments and get serum for home treatment for free, and the other one is a free consultation for any cosmetic treatment spending a total of $8 a day (in the Czech Republic).
However, the lead quality is not what I expected. Out of these 42 leads, only one came into the salon and the rest were scheduled but didn't come or cancelled it. What we do is that I get her lead, I'll send her the info about the lead and she calls them and books them in herself. I don't think it's the lady's mistake, but there's just something off with the quality of people filling out the FB opt-in.
I was thinking about some additional age or interest targeting. Potentially putting more questions into the opt-in since now they currently fill out their name, email, phone and city. I'm not sure.
Do you have any tips on how to get a better percentage of people that come into the salon? I am extremely grateful for any tips and would love to connect. 🙏 Thank you!
