I have reviewed both modules and truly appreciate the amount of work that has gone into them. I must say, you’ve done an excellent job, and I understand how challenging it can be to develop such comprehensive materials and bring them to this stage. That said, I believe there are a few minor areas that could be refined to further enhance the course. I’ve made notes as I went through the content systematically. The list is probably too detailed to include here, so I’m happy to send it to you via email or whichever method you prefer.

Accident Investigation
Have you ever wondered why preliminary accident investigations are concluded relatively quickly, whereas the final report often requires months or even years to complete? When an accident occurs, the first priority is to determine what happened. This part is relatively straightforward — with today’s technology, such as the Cockpit Voice Recorder (CVR) and Flight Data Recorder (FDR), along with other available evidence, investigators can usually establish the sequence of events quite quickly. However, uncovering why the accident happened is a far more complex and time-consuming process. This stage often involves a deep examination of human factors — psychological, physiological and organisational influences that may have contributed to the event. I’ll leave the discussion here for others to share their insights and elaborate on the specific factors that can play a role.

Absolutely, Andrew. Countless factors can contribute to the chain of an occurrence. My goal is to explore these interconnected issues and bring them to the forefront, so we can draw meaningful lessons for the future. For instance, if we focus exclusively on a single topic like fatigue, the audience might overlook other important contributing factors.
