I just wanted to share this intake form I designed for my business. I wanted to give others ideas if they are new or looking for an option for their intake form. I incorporated suggestions Dan has given in the course and added some based on my specific Live Scan System. By no means is this the 'standard' I am sharing this in hopes it may help someone? 2-page document. Side 1 is for the customer. The other is for In Office Only. The customer will never see this form again, however, is used to add notes to prompt conversation for a thank you card or any type of follow-up that helps staff remember that customer specifically.
