A lift was planned for a known load weight and configuration. Just before lifting, additional items were added “to save time.” The lift plan was not updated, and no reassessment took place. The crane operated closer to capacity than intended.

Any change to load weight, size, or configuration requires the lift to stop and the plan to be reviewed and reapproved.
