KEY TASKS
────────────────────────
Key Tasks: Assess and manage complex care plans, coordinate with providers and caregivers, monitor member progress and revise plans, identify and address care barriers, conduct telephonic and digital outreach, document care activities for compliance, educate members and families, analyze care trends, support and precept clinical new hires

QUALIFICATIONS
────────────────────────
Qualifications: Registered Nurse licensure, degree from an accredited school of nursing or BSN, 4–6+ years related nursing or care management experience, experience with complex case management, strong communication and documentation skills
