A one-page note for your next appointment
Ever walked out of an appointment and remembered the one thing you meant to ask?
Here is a simple one-page note you can fill in beforehand:
1. Your concern, in your own everyday words.
2. When it began, how it affects your day and what you want to ask.
3. Medicines, supplements or devices you use, to talk through with your clinician. NCCIH encourages telling your health care providers about any complementary approaches you use: https://www.nccih.nih.gov/health/are-you-considering-a-complementary-health-approach
4. Your most important question at the very top.
Then bring it along, on paper or on your phone. A shorter version still counts.
This page is just for you to get your thoughts in order. Please keep your personal details for the note itself, not the comments.
Save this post so the layout is ready for your next conversation.
