Cleaning up the list

So, I was wondering, how do you all keep your list clean? I have been using Mailerlite the past year and they had the option to see how often someone opened an email and / or clicked on a link inside the email, but that's all. I moved to Kartra a few weeks ago and they have a filter to filter out people who have been 'not active' for the past 90 days or more. But there is nowhere to be found what this 'not active' means.

What are your filters to remove the subscribers that aren't active? When are they no longer of 'use' to you? And when do you consider your list to be 'clean'? And last, how often do you clean your list?
