Hi everyone! I’m Natasha Jackson, RN, CEO of iCareRN, and I’m so excited to welcome you to our community. 😍 Let me share a bit about myself and how I got here. I’ve been a nurse for over 20 years, and about two and a half years ago, I took a leap of faith and started my own care management business. At the time, I had no formal experience in care management, just my clinical expertise, a passion for helping patients, and a drive to make a difference. Fast forward to today, I’ve built my business from the ground up, signed my first six-figure contract this year, and learned so much along the way. It wasn’t easy, and I certainly didn’t do it alone. With the guidance of coaches, valuable resources, and a lot of hard work, I gained the confidence to turn my vision into reality. That’s proof to me that with the right mindset, mentorship, and resources, anything is possible. Now, my mission is to share what I’ve learned to help other nurses avoid the pitfalls I faced and build successful, sustainable care management businesses of their own. This community is a space for us to grow together, share ideas, and support each other in this journey toward entrepreneurship. If you've been inside this community learning about care management but haven't taken the next step toward actually starting your business, this is your opportunity. The Physician Fee Schedule and Modifier 25
This video breaks down one part of the proposed 2027 Medicare Physician Fee Schedule: Modifier 25, what it means, and why providers are raising concerns about the proposed payment changes. But my bigger message to everyone in this community is simple: please comment on the proposed rule. You do not have to understand every page of the Physician Fee Schedule. Use an AI tool to help you summarize the proposal, identify the sections that affect the work you do, then go back to the actual CMS proposal and submit a thoughtful comment based on your experience. As nurses, this is also an opportunity to speak up about the role we play in care management. I want CMS to hear from more nurses about the need for payment policies that recognize our work and create pathways for nurses to be paid independently for care management services. If something in the proposed rule affects your patients, your practice, your profession, or the way you deliver care, say something. The deadline to comment on the proposed 2027 Medicare Physician Fee Schedule is September 14. Please comment. Your voice matters in this process. If you are a nurse building a care management business and Remote Patient Monitoring is part of your business model, I need you to pay attention to this proposed change. CMS is proposing a change to Remote Patient Monitoring for 2027 that could directly affect nurses who contract with physician and nurse practitioner practices. Under the proposal, Medicare payment for the clinical-staff portion of RPM would be limited to clinical staff employed by the billing practitioner or practice, rather than contracted clinical staff. Think about what that means if your business model is to partner with a medical practice as an independent RN. You may have access to the EHR, document directly in the patient's medical record, follow the practice's protocols, communicate with the physician or nurse practitioner, monitor patients between visits, escalate abnormal findings, and function as part of the patient's care team. But you are still a contractor. If CMS finalizes this proposal as written, that could affect the ability of practices to use contracted nurses to provide billable RPM clinical services. CMS has legitimate concerns about fraud, inappropriate billing, and third-party RPM companies that may not be sufficiently integrated with the patient's healthcare team. But should the solution be eliminating contracted clinical staff? Or should Medicare establish stronger standards for contracted clinicians, including EHR access, documentation, supervision, clinical integration, escalation protocols, and accountability? If you are building a business around partnering with practices to provide RPM, this is your opportunity to tell CMS what this proposal could mean for your business model and the patients you intend to serve. Don't wait until 2027 to find out that the policy changed. Comment while it is still proposed. The deadline is September 14th.
