I want to start by saying thank you for being here and for your patience with me this week.
I won’t be live for our weekly session tonight, instead, I’m sharing these slides to make sure you still get value and clarity around CCM.
Today, we’re going to focus on Chronic Care Management.
What it is, who it’s for, what’s changed, and how it fits into care management practice today.
The information I’m sharing comes directly from the Medicare Learning Network, the MLN document, updated June 2025. This is the most current guidance CMS has released on Chronic Care Management, and it’s meant to be a practical resource you can use to understand how CCM works in real practice.
As you review, I want you to think about how CCM shows up in the work nurses already do, and how understanding this service better can help you position yourself more intentionally going forward.
