A properly played coven of hags that follow a green vampiric dragon, with necromancers, evil fey and goblinoid followers. The Vampiric Green Dragon studies the party for months using the coven and the necromancers resources before revealing itself after the coven is overturned.

I always end up tapped out in my TTRPG our DM doesn't do rests very much I went months in same dungeon about 8-9 weeks of gaming every week with no rest no resources ended up doing 1 melee attack per round with my wounded cleric with minimal effects until we got out to a tavern then rested leveled up only to go back to the grind!

My party just finished a cave dungeon ragged and beat up with little to no spells low hit points we get to the cave exit to find a horde of Goblins ready to attack at front entrance. The party retreats, I make the entrance impossible to pass through without being single file with rubble furniture while the Goblins argue about their strategy. I am able to make about 5 fall back points where I can hold off the hoard with my Paladin. I rolled 7 natural 20 hits in a row killing every Goblin stupid enough to poke his face through my single file obstructions. After the 7th natural 20 instant kill hit the goblins hoard failed moral and ran away! Best rolls I ever made in 40 years of dnd!
