This will happen more times that you'd expect as it has happen to me quite a few times. Sometimes people just don't realise that they've bought it before. It just shows that they really like your stuff. But instead of a refund I just give them another digital file of similar price that is close to what they bought before that is also selling. That's adding value. All of them are really appreciative of this. I don't like to refund because I feel it may trigger Etsy to think my customer service is not up to scratch and that may affect my best seller status. Just my thoughts, not proven data.
