Paediatric Pearl: Cough! Could It Have Been the Ear? 👂🩺
Cough is one of the most common symptoms we see in children… but every so often, a case reminds us to think outside the usual respiratory box. Todays paediatric pearl focuses on an important (and easily overlooked) trigger: the ear. Children with otitis media, effusion, or even mild middle-ear inflammation can present primarily with persistent cough, thanks to the shared sensory pathways of the vagus nerve. In some cases, the cough resolves only when the underlying ear issue is treated. It’s a great reminder that: Not all coughs are respiratory in origin A quick otoscopic check can change your entire management plan Holistic assessment remains key in paediatrics A simple ear exam might just be the missing piece of the puzzle. Have you come across a “respiratory” complaint that turned out to be something entirely different? Share your experiences below!

Really appreciate this reminder. I've seen a few cases where a persistent cough didn’t fit the usual respiratory pattern, and an otoscopic exam completely shifted the diagnosis. The vagus nerve connection is something we don’t always think about in the moment, but it makes all the difference. Great pearl, holistic assessment truly is key in paediatrics.

The Power of Case-Based Discussions: Learning Through Real Experience
Case-based discussions (CBDs) are one of the most effective ways to bridge the gap between theory and real-world practice. By exploring real or simulated cases together, we move beyond textbook knowledge to understand how decisions are made, what challenges arise, and how different approaches can lead to different outcomes. Why they’re important: 🧠 Deeper understanding: Discussing actual cases helps solidify knowledge and reveal nuances that standard teaching might miss. 🤝 Collaborative learning: They encourage sharing perspectives and learning from each other’s reasoning. 🔍 Reflective practice: Reflecting on cases helps us recognize what went well, what could be improved, and how to apply those lessons next time. 📈 Better decision-making: Working through complex, real-world scenarios builds confidence and clinical (or professional) judgment. If you’ve been part of a valuable case-based discussion, what made it impactful for you? Share an example or insight below — it could inspire how others approach their next case discussion.

Love this. CBDs are where so much real learning happens. I’ve found that hearing how others break down a case has helped me grow my own judgment and confidence. It’s amazing how one scenario can lead to so many perspectives.
