Perfect Pt Journey
Day 0 - Initial contact -Pt contacts office from a source. CA takes details, takes deposit, sends initial paperwork to be completed. CA gives location of practice and books NP and ROF appts.
Day 1 - Initial Consultation - Pt enters practice, CA welcomes, DC greets and welcomes, explains process for the Init cons. Takes Hx, performs assessment and then handover to CA. CA confirms ROF appt
Day 2 - ROF - CA welcomes Pt, DC - welcomes back, explains ROF visit, performs ROF includes care plan, costs, consent for plan/costs and informed consent for Rx, Adjusts pt then - handover to CA - CA books care plan appts including PE and reassessment, discusses financial policy. and collects payment. Care plan given or emailed to pt..
Day 3 - DC does 3 legged stool - schedule of care, removing bad habits, importance of stretches when they are given, Adjust pt
6th visit - Progress examination - subjective and objective (usually positive findings from Init) to determine if pt on track. DC adjusts and gives mini report.
12th visit - Reassessment - full reassessment , DC adjusts - mini report. Handover to CA - explain new care plan and financials. CA books new care plan and discusses finances for that visit. Care plan given or emailed to pt.
12th visit of new care plan - as above and continues for every new care plan from this one.
