Why Claude Forgets — and How to Fix It
Claude Code reads a file called CLAUDE.md at the start of every session. It's how you give Claude context about your project: what you're working on, how you like to work, decisions you've already made. The problem is that most people set it up once and never update it. So over time, it drifts. Claude has the original context but not the learnings from the last 10 sessions. There's a free plugin called claude-md-management that helps with this. It has 2 skills: - /revise-claude-md: reviews your session and suggests updates to the file at the end - /claude-md-improver: grades your CLAUDE.md and flags what's outdated or missing I run the first one after almost every session.
