Most training programs teach you how to pass exams, but they don't always prepare you for the complexity of real-world psychiatric practice. That's why I created this community. Sometimes the most valuable learning comes from taking a question you’ve encountered in practice and slowing down to really think through it. So, I want to hear from you. What is one question you’ve been wanting to ask about psychiatry or clinical practice? What Would You Do? A 32-year-old patient comes to your outpatient clinic reporting worsening anxiety and insomnia. They say: “My mind won't shut off. I'm sleeping only 3–4 hours a night, but strangely, I don't feel tired.” They also report increased energy and productivity at work. Before jumping to a diagnosis or medication, what would you want to know next? Think about: 🧠 Mood history 😴 Changes in sleep patterns 💊 Current medications 🍷 Substance use 👨‍👩‍👧 Family psychiatric history 📅 Timeline of symptoms There is no need to have the “perfect answer.” The goal is to practice your clinical thinking. 💬 What questions would you ask next?

🧠 Clinical Pearl: Before You Change the Medication, Change the Question
When a patient isn't improving, it's easy to immediately think: “Should I increase the dose?” “Should I switch medications?” “What should I add?” But good psychiatric practice starts before the prescription. Before changing the treatment plan, take a step back. Ask yourself: Do I fully understand what is happening with this patient? Revisit the timeline. Reassess the symptoms. Look at adherence, substances, medical contributors, side effects, and the differential diagnosis. Because sometimes the problem isn't that the medication is wrong. The problem may be that we haven't asked the right questions yet. 💡 Clinical reasoning comes before medication selection.
