This form directly determines your child’s next training focus.

• One form per weekend
• If multiple games were played, give an overall average
• Be honest and specific
Deadline: Monday at 12:00 PM sharp
Form is sent Sundays at 8:00pm and closes Monday at 12:00 PM.
