Care Logs: Your Agency’s First Line of Protection
Care logs are more than daily notes – they are legal proof of care delivery.
Every visit should document:
  1. Date and time of visit.
  2. Caregiver name
  3. Tasks completed (bathing, grooming, and meal preparation)
  4. Client condition or changes. 
  5. Any incidents or unusual observations. 
Incomplete logs create serious risks for agencies.
If a family questions whether services were delivered, your documentation becomes your evidence.
