RarePatientVoice pays their users $2/minute for health-related surveys, and pays you $10 per person who signs up with your link and verifies having health conditions. Not all health conditions will count though. Some very common ones such as generalized anxiety disorder won't count towards helping you get a successful referral... but at $10/lead and $2/minute, you can't really go wrong.
