Phantom Coalition
Bilateral radiographs w/ weight-bearing and right ankle MRI performed https://www.cmrad.com/cases/1735748974 Patient was subsequently taken to the OR for a calcaneonavicular coalition. The issue is that I"m having trouble seeing it on the pre-operative images :S

Chronic ankle instability following ankle sprain
Wanted to share this case because it has nice follow-up from a case that Dr. Jbara eloquently discussed on her forum https://www.cmrad.com/cases/1152399287 Here is the original case discussion: https://vimeo.com/913815661/79eb159a23?share=copy There have been some healing changes of the low lateral ankle ligaments. Unfortunately, the patient has developed an osteochondral lesion of the anterolateral talar dome—likely secondary to repetitive microtrauma from instability related to ligamentous laxity. Doesn't look like there is instability of the OCL, so I'm guessing the referring provider will opt for conservative management initially Any thoughts on the case and prognosis/management @Marlena Jbara and others?

@Im Beg thank you for your support- this is a complex case- i dont think thats the capsule but rather fibrotic bands and hemarthrosis - see the video case below and the articles attached " The hemarthrosis associated with a lateral ligament complex injury predisposes to a posttraumatic synovitis in the anterolateral gutter, which, over time, may become coalescent and undergo hyalinized fibrosis (Fig 6) (7,12). The hyalinized fibrotic tissue may be round/ linear in morphology or become triangular/meniscoid in shape if it molds to the contour of the anterolateral gutter"

Is this a osteochondroma / Hoffa disease / something else ? And is it recommended to ask a MRI for this ?

I can see what you mean about the mri being xtra since the arthritis is severe- but if there is new pain and trauma- one would have to make a clinical call whether degree of edema precludes weight bearing. Now would like that mri to see what’s there!

@Neil Shaba no radiation and gives me more qualitative information on ligaments menisci and regarding degree of edema. Could guide me to determine weight bearing status. Many insufficiency fractures underestimated on ct. @Neil Shaba
