If I were on clinicals with an incontinent patient, my first step would be a thorough skin assessment, really looking for any early signs of moisture-associated skin damage. From there, I’d focus on getting them clean, dry, and comfortable, and then applying a protective barrier right away. I’ve found that being proactive with skincare, combined with repositioning and reducing moisture exposure, makes a huge difference in keeping patients safe and preventing complications before they start.
