What to do when Claude consistently ignores information in the dna documents?

I have been using Claude to help me with social and each time he references the dna, the business dna and my voice skill before writing. Often he misses that a friend helped me in the airport years ago and he puts it was a stranger. Each time i point it out and each time he acknowledges the mistake and we update the skills so hopefully it will not happen again.

So far it is still happening. So, just a shout out to all - make sure you check what Claude is writing. He does make mistakes. If anyone knows how to fix this - please let me know.
